1: The comprehensive assessment

As part of its conditions of participation, the Center for Medicare and Medicaid Services (CMS) requires certified home health agencies (HHAs) to carry out comprehensive assessments at certain time points during a patient's period of home health care. A comprehensive assessment is an evaluation of a home health patient's physical and cognitive health status and must identify the patient's need for home health care services, as well as addressing the patient's medical, nursing, rehabilitative, social, and discharge planning needs. Specified OASIS data items must be incorporated into the comprehensive assessment.

Challenging clinical items

Home health agencies must carry out comprehensive assessments at certain times during a patient's period of home health care.

Based on the comprehensive assessment, a plan of care must be created and measurable patient goals must be established and reviewed. An example of a measurable patient goal is:

  • "In six weeks, the patient will walk ten steps safely with the walker."
After a comprehensive assessment, the HHA needs to complete form HCFA- 485. Having a physician signed 485 is required before the HHA can bill the final claim for Medicare patients or bill any visit for other payer sources.. The form is a plan of care and must list skill, frequency and duration of all types of visits ordered by the physician, list of measurable goals, medications, diagnosis with date, rehabilitation potential, discharge plans and documentation of homebound status. When a new patient is referred to a home health agency (HHA), a comprehensive start of care assessment incorporating certain OASIS data items must be carried out. The initial assessment visit defines the start of care date for this patient. During this visit:

  • The patient's health status and use of medications must be reviewed
  • Measurable patient goals must be developed
  • The patient's homebound status and need for home health care services must be verified for Medicare (and, in some states, Medicaid) patients only.
