1: Overview of OASIS

OASIS is an abbreviation for Outcome and Assessment Information Set - assessment information about home health patients that is used to quantify patient characteristics and measure patient outcomes.

OASIS is a set of patient assessment data items that addresses:

  • Demographics (including age, ethnicity, and living arrangements)
  • Support systems (care assistance from family or friends)
  • Health status (physical and psychological conditions)
  • Utilization of healthcare services (including inpatient and emergency care).
According to the Health Care Financing Administration's (HCFA) conditions of participation, OASIS data must be collected by certified home health agencies (HHA) as part of a comprehensive assessment at certain time points during a patient's period of home health care. Comprehensive assessments are conducted:

  • At the start of care
  • At scheduled follow-up visits, every 60 days from the start of care date
  • Whenever the patient's condition changes significantly
  • Whenever a patient is transferred to an inpatient facility (no visit required)
  • When home health care services are resumed after an inpatient stay of 24 hours or more (except for diagnostic testing)
  • When a patient is discharged from home health care (no visit required for an unplanned discharge)
