Information Collection and Use

CEU4U is the sole owner of the information collected on this site. We will not sell, rent, or trade this information in ways different from what is disclosed in this statement. CEU4U collects information from our subscribers at several different points on our website.

Testimonials

We post customer testimonials on our website which may contain personally identifiable information such as the customer's name. We do obtain the customer's consent prior to posting the testimonial and name.

Registration Form

Our site’s registration form gathers contact information (i.e., your name and e-mail address), demographic information (i.e., your zip code,) and unique identifiers (i.e., the last 4 digits of your social security number and professional license number). Your contact information will be used to get in touch with you should we have a question about your registration. CEU4U occasionally uses your contact information to send you important updates about new features on the Web site. You may opt-out of receiving future mailings; see the choice/opt-out section below. Demographic and profile data is also collected at our site. We use this data to tailor your experience at our site, showing you content that we think you might be interested in and displaying the content according to your preferences. Unique identifiers (such as last 4 digits of your social security number and e-mail address) are collected to verify your identity and for use as account numbers in our record system. CEU4U does not sell, rent, or trade your personal information to third parties. If you have been referred by a member, you will be asked to provide that member's Login ID or email address.

Refer a Friend Program

This is how our Refer a Friend Program works: When a user registers, the user is asked if they were referred by a friend. If the answer is yes, we ask for the friend's e-mail address. The friend that made the referral will automatically receive one free continuing education hour when the friend that they referred takes a course. We do not collect information from the friend who was referred.

You may also click on the Free CE Hours link on the menu, and provide your friend’s email address. We will automatically send your friend a one-time email inviting him or her to visit the site. CEU4U, Inc. stores this information for the sole purpose of sending this one-time email and tracking the success of our referral program.

Your friend may contact us at support@ceu4u.com to request that we remove this information from our database.

Order Form

Our site uses an order form for you to purchase continuing education certificates. We collect your contact information (i.e., your e-mail address) and financial information (i.e., your account or credit card numbers). Your contact information is used to contact you when order information is inaccurate or if services beneficial to you are available. You may opt-out of receiving future mailings; see the choice/opt-out section below. Financial information that is collected is used to bill you for your continuing education certificates. A third party, NOVA, is used to process and verify your credit card information. When you purchase a course, we will share your credit card information with NOVA so that it may provide its service. NOVA is prohibited from using your personally identifiable information for any other purpose. CEU4U.com does not retain credit card information.

Feedback Form

The feedback form is a voluntary form that is used to improve our web site. By compiling this data we are able to improve customer satisfaction by responding in a timely manner to frequently asked questions. The following types of personally identifiable information are collected on the feedback forms: name, email and/or phone.

Course Evaluation Form

At the successful completion of a course, each customer is required to complete a course evaluation form. The forms are used to measure customer satisfaction of on-line learning and to determine whether the course objectives are satisfied. This form is used for internal purposes only. The course evaluation form does not collect any personally identifiable information; however, the user can be identified because the evaluation is connected with the test.

CEU4U Record Keeping

CEU4U maintains a participant database containing a record of completed tests, test scores, and cumulated certification hours for seven years. It is an accreditation requirement to document a user’s participation in courses designated for accreditation. We do not use this information for any other purpose and we do not provide or sell this information to any third party.
